Link: Alternative Income Form

Dear Parents and Guardians,

FUSD needs your help to maximize state and federal funding for educational programs that directly benefit district students.

Some of the funding to the school district is determined based on the demographics of the student population. Funding may be increased depending on the number of low-income students the district has. In order to determine if our students qualify for specific state funded programs, FUSD households are asked to complete an online Alternative Income Form. The information you provide is kept confidential.

The online form only takes a few minutes to complete.

If you need assistance completing this form please call Christina Trujillo at 510-657-2350 extension 12637
